Consent to physiotherapy treatment and data collection

Prior to your initial physiotherapy appointment, you will be asked if you are willing to consent to an initial assessment and consequent treatment sessions.

 

Your assessment and proposed treatment plan will be fully explained to you and you will have the opportunity to ask any questions that arise. Should you have concerns or questions about any recommended treatment, you must inform your therapist immediately.You have the right to withdraw your consent for assessment and treatment at anytime.

 

During your physiotherapy visit, it is often necessary to expose the body part in need of treatment and come in to close contact with you. Every effort is made to preserve modesty and keep you comfortable. Please communicate to your therapist if you have any concerns during the treatment. You are welcome to bring a friend or family member with you during your treatment session.

 

As with all forms of medical treatment, there are benefits and risks involved with physiotherapy. There is no guarantee that the proposed course of treatment will improve your condition and that is possible, although unlikely, that the course of treatment may cause additional discomfort or aggravate your condition.It is your responsibility to fully cooperate and participate in your rehabilitation process. If you choose not to participate, you must inform your therapist immediately.

 

Please note that we do record elements of personal data. This data is recorded on secure password protected files that are accessed by only authorised personnel. If wish to decline to having any data recorded, please indicate on the data collection form.
